Free Community Support Available Through GUIDE

Episcopal Senior Life Communities‘ (ESLC) Community Dementia Program is excited to announce participation in the new Medicare Guiding an Improved Dementia Experience (GUIDE) Program. Through GUIDE, the Community Dementia Program expands its compassionate, community-based support for people living with Alzheimer’s disease and other dementias and their families. The program offers benefits at no cost to individuals who have traditional Medicare A/B.

Through this program, ESLC partners with Rochester Regional Health’s Unity Geriatric Associates to provide virtual assessments, in partnership with our trained dementia specialists to create individualized care plans, education and programming. This comprehensive approach gives families access to the
following services and supports:

  • Personal Care Navigation
  • Health and Wellness Programs
  • 24/7 Support Line
  • Respite Services
  • Coordination with your Medical Team
  • Caregiver Education and Support
